While assembling of HFO purifier 2/E not taken proper boiler height while assembling of purifier during out of HFO purifier while doing de sludge operation the bowl hood got damaged and got heat purifier to the 4/E & E/O

Corrective Action/ Lessons to be learnt:
Always measure bowl that before and after dismantling & assembling of purifier
Proper PPE
Refer manufacturers manual carefully
Consult senior officer in case of any doubt
