Summary:
Air entered into generator jacket cooling water system leads to black out during piolatage.

Corrective Action/ Lessons to be learnt:
Vessel not maintained nitrite level as per marker instruction so all nitrite level to be maintained as per marker instruction
During decarb nozzle holder to be replaced as running hrs given in manual 8000hrs
Senior engineer to make sure all cylinder heads to be pressure tested during decarbonisation.

Incident:
Vessel taken over from another management and departed from Coterie Brazil to Santos Brazil via Amazon river. During pilotage suddenly CW pressure dropped jacket cooling temp increased and tripped. Cooler tubes got damaged, and system got airlocked. Cooler replaced and situation normalised.
